At La Peruca, we hear one of them particularly often:
"Will my health insurance pay for a human hair wig — and what will it cost me?"
The honest answer is: it depends. In this guide, we explain in simple terms when health insurance companies will cover the costs, which price ranges are realistic, what specifically TK, BARMER, AOK, and other insurers pay — and what policyholders should know before they submit a claim.
Who is eligible for a wig on prescription?
People with statutory health insurance are entitled to a hair replacement if the hair loss is medically related. This includes, for example:
- Chemotherapy for cancer
- Alopecia areata (patchy hair loss)
- Alopecia totalis
- Alopecia universalis
- Scarring alopecias
- Burns or injuries to the scalp
- Other conditions involving permanent hair loss
As a rule, a medical prescription for hair replacement is required. With this prescription, you can apply for coverage from your health insurance provider.
What does a human hair wig cost — and how much does health insurance pay?
This is the question we are asked most often. Here are some realistic guidelines:

What exactly does health insurance pay?
Statutory health insurance providers work with fixed amounts – this is the maximum amount they will cover for medically necessary hair replacement. If the costs of the selected wig exceed this amount, the policyholder pays the difference as a personal contribution.
The following coverage amounts are often used as a guideline:
- Synthetic hair wig (short-term use): approx. €300 to €500
- Synthetic hair wig (long-term use): approx. €500 to €900
- Human hair wig (long-term use): approx. €720 to €1,500
In addition, there is generally a statutory co-payment of €10 per medical aid (for policyholders without an exemption).
Important: The actual cost coverage depends on the individual health insurance company, the applicable supply contract, and the medical prescription. We will clarify your individual entitlement to benefits before you submit your application and inform you transparently about potential coverage and any possible co-payment.
How do I get a wig through my health insurance?
The path to obtaining coverage is usually simpler than many affected individuals initially assume.
1. Obtain a medical prescription The first step is a prescription from your attending physician.
2. Consultation with a qualified hair studio During a personal consultation, we will determine which type of hair replacement is appropriate, what options your health insurance provider offers, whether synthetic or human hair is suitable, and what documents are required.
3. Submit a cost estimate to your health insurance provider Depending on the insurance provider, approval will be granted, or you will be provided directly.
4. Individual fitting Once approved, the wig will be fitted and customized to your personal requirements.
Does health insurance cover human hair wigs?
In principle, statutory health insurance companies cover the costs for medically necessary hair replacement. However, whether a human hair wig is covered depends on your diagnosis, your health insurance provider, individual medical requirements, and current contractual regulations.
Many policyholders assume that a human hair wig is automatically paid for. In practice, this is not always the case.
Human hair or synthetic hair - which is the better choice?
Many clients come to us specifically requesting a human hair wig. That is understandable: human hair offers a particularly natural appearance, individual styling options, natural movement, and great flexibility in everyday life.
Nevertheless, human hair is not automatically the best solution. Modern synthetic hair wigs have evolved tremendously. Today's high-quality models look very natural, are comfortably lightweight, and maintain their style even after washing. Our experience shows: The best care is not automatically the most expensive one. The deciding factor is always your personal situation.
What applies to TK, BARMER, AOK, DAK, and other insurance providers?
TK and BARMER (vdek insurers)
Techniker Krankenkasse, BARMER, DAK-Gesundheit, hkk, HEK, and KKH belong to the Association of Substitute Funds (vdek). For these insurance companies, the following applies:
- Synthetic hair is generally considered the standard provision
- A human hair wig is not automatically approved
- For human hair, you generally need medically documented proof of an allergy or intolerance to synthetic hair fibers. A simple note on the prescription is not sufficient.
Many insured persons are not aware of this and are surprised when their health insurance provider initially points them toward synthetic hair options. This is why we always recommend an individual consultation before submitting an application.
AOK
AOK is the largest health insurance provider in Germany—and for many of our customers, it is the most relevant point of contact. At AOK as well, synthetic hair is generally considered the standard form of care. Regulations may vary slightly depending on the regional AOK (AOK Bayern, AOK Berlin-Brandenburg, AOK Nordwest, etc.). However, the general rule is: anyone seeking a human hair solution also requires medical justification with AOK—usually proof of intolerance to synthetic hair fibers or a long-term need for care.
We help you prepare the right application for your specific AOK region.
Special features for chemotherapy
One of the most frequent questions is: "Will the health insurance cover a human hair wig during chemotherapy?"
In most cases involving chemotherapy, a so-called short-term solution is provided—the background being that hair loss is usually temporary and natural hair growth often resumes after treatment is completed.
For this reason, high-quality synthetic hair wigs are predominantly used for chemotherapy. This is due to several reasons:
- Rapid availability
- Low maintenance requirements
- High suitability for everyday use
- Short treatment duration makes more expensive alternatives economically unjustifiable
Chemotherapy generally does not provide a basis for a claim to long-term human hair replacement.
Many patients are surprised at how natural modern synthetic hair wigs look today—and feel much more confident after a consultation.
Permanent hair loss: When is a human hair wig an option?
The situation is different for permanent or long-term hair loss—such as with alopecia totalis, alopecia universalis, scarring alopecia, or permanent hair loss following illnesses or treatments.
In these cases, health insurance providers refer to this as long-term care. Many affected individuals wear their hair replacement for years or permanently—consequently, the requirements for comfort, naturalness, practicality for everyday life, and styling options increase.
Under certain conditions, a human hair solution can therefore be considered.
Longer wear time for human hair
In cases of permanent hair loss, some health insurance providers take a longer usage period into account for approved human hair solutions—due to their greater durability. This means the replacement generally needs to be renewed less frequently than synthetic hair.
What costs must the insured person cover themselves?
The amount of the personal contribution depends on the chosen wig, individual customizations, special hair lengths, additional comfort features, and the amount of the health insurance subsidy.
As a rough guide:
- For a synthetic hair solution within the fixed-price allowance: personal contribution often between €0 and €200, depending on the insurance contract
- For a human hair solution: personal contribution from €500 to over €2,000 possible, depending on the selected model
-
For an approved long-term human hair solution: higher insurance subsidy, but often still a personal contribution
We will provide you with a transparent cost estimate before submitting the application—so you won't face any nasty surprises.
Our practical experience
Many people come to us wishing for a human hair wig. However, after a detailed consultation, quite a few decide consciously in favor of a high-quality synthetic hair solution—because modern models often look much more natural today than many expect.
At the same time, there are customers for whom human hair is the better long-term solution.
Therefore, there is no standard answer. Every person, every diagnosis, and every life situation is different. That is precisely why we take the time for an individual consultation and, if desired, will support you with all questions regarding prescriptions, cost estimates, and health insurance.
